Background and aims: To investigate the value of acetic acid visualization of the cervix in early detection of cervical pre-malignant & malignant in developing countries and the comparison of cytology and visual inspection.
Methods: Visual inspection was performed on 468 women aged 20-40 years who had undergone speculum examination. First cytological smear was performed, and then naked eye inspection of the cervix after application of acetic acid.
Patients with suspicious acetowhite lesion were immediately referred for colposcopy & biopsy.
Thus histology was obtained from all patients with a positive acetic acid test.
Results: 43 women with positive acetic acid test were referred for colposcopy and biopsy.
3 patients (6.97%) had cancer, 8 (18.60%) had squamous intraepithelial lesion (SIL) (7 low SIL,
1 high SIL), and 32 (74.41%) had benign finding.
11 of the 20 women with positive cytology and positive acetic acid test had low and high -grade
SIL and cancer.The average positive predictive value of visual inspection was 34%.
Conclusions: In 94.3% the cytology exam and the acetic acid application produced exactly the same result. Although the false positive of simple visual inspection is more, as a result it causes incongruous colposcopy, but by considering of the low cost of simple visual inspection, needless to skill and specialist and immediate result, in remote and deprived regions this method is preferable.
