| چکیده | Background: Chronic Disease Self-Management Program (CDSMP) is one important way to improve and enhance self-care in patients diagnosed with different chronic conditions which is well-known in the world. This study was aimed to evaluate the effectiveness of the CDSMP on the health status indicators of patients with coronary artery bypass graft (CABG) surgery. Methods: In this study, as a before and after approach, the program was conducted on 46 patients with a history of CABG surgery referred to the cardiac rehabilitation unit of Imam Ali hospital in Kermanshah. Samples were selected with using a convenience sampling method. The CDSMP was carried out one session per week for six weeks. Data were collected three times before, immediately after and three months after the program using a General Self-Efficacy Questionnaire and a Health Status Measurement Tool. Data were analyzed using Chi-square test and ANOVA with repeated measures by using SPSS software version 21. Results: Data analysis showed that most participants were men and married. The impact of the program on public health, daily activities, and participants‘ pain feeling was significant (p<0.001), but a significant relationship was not found regarding the overall health status, energy or fatigue, feelings of well-being and social performance. Conclusion: This study demonstrated the effectiveness of CDSMP on some indicators of the self-management in patients undergoing CABG surgery.
